Formally, 1. Three US presidents have been mayors, and he is one of them.
Many people would write or say 2, though.

I don't think a capital letter is desirable here on "president"
or "mayor".

As to the verb, "have" is correct; "US presidents" is the subject.

As to the last part, "a mayor" and "mayors" are both correct. It's
"a mayor" if you mean to start with the expression "US president who
has been a mayor" and pluralizing it. If you mean the "who" clause
to apply collectively to all three of those presidents, then you want
"mayors".
